COVID-19 Vaccination Documentation Upload

On this page you can upload your covide vaccine certficate(s).

Enter your name and email. Then select your Covid Saftey Officer from the list.  You can find out about them on our Covid Readiness Page. If you don’t know your officer, select “Do not know”.

Now push the “Choose File” button for each image you want to upload. You may upload up to three images. 

When you are ready click “Submit” and your files will be uploaded to the company storage.
